In the chapters in this section of the collection, practice is deconstructed and presented in a way that highlights the hidden or neglected competencies that make clinical practice safe.

Studies have also been conducted to further understand how organizational cultures impact upon patient safety, for example developing tools to characterize and measure safety cultures in primary care and secondary care (Pronovost and Sexton 2005, Kirk et al. 2005). In addition, studies have examined the best ways to implement incident reporting and encourage staff participation, for example through undertaking comparative analysis of different health care sectors and other industries (Helmreich and Merritt 1998).
